Replacing Lost I.D. Cards

You may never lose your wallet or have it stolen. But suppose you did. Would you know your credit card number? Would you know what pieces of identification you were carrying? Would you know how to replace your social insurance card?Credit or bank card?

The following tips will help you get organized and protect yourself against loss or theft. Also included are detailed instructions on how to replace identification cards when they are lost or stolen.

Start by taking a few minutes to write down the identification numbers of all your I.D. cards. Once your list is complete, put it in a safe place for future reference.

If you drive and own a car, always carry your driver's licence and car registration with you. In case you get into an accident, you should carry your Manitoba Health Insurance card as well.

Other identification such as your birth certificate, blue cross, bank or credit cards should be kept at home, except when you intend to use them.

If your wallet is lost or stolen, report it to your local police or RCMP detachment immediately. If you lose a credit card, bank book or card, or travellers' cheques, report it the bank as soon as possible.

Birth Certificate

Replacement cost: $25.00 per certificate.

Cheque or money order should be made out to the Minister of Finance.

Contact the Vital Statistics office. Give your full name, date and place of birth, father's full name, mother's maiden name and the place of birth for both parents. Include your signature and complete mailing address, and state whether you wish the wallet size or long form birth certificate.

If you were not born in Canada, the Vital Statistics office can provide you with information on where to call or write for a foreign birth certificate.

Social Insurance Card

If you go in person, bring your birth certificate and another piece of identification with you. Fill out the form required to get a new card. Call the above number for information on how to apply for a new card by mail. Application form is also available on-line at Service Canada.

You must fill out an application form. If you go in person, a temporary card will be issued immediately. If you mail your application form in, you should receive a new licence in the mail in three weeks.

Winnipeg Transit Picture Card

Call Winnipeg Transit's Lost and Found at 986-5054 to find out whether your wallet and/or picture card has been turned in. If not, another picture can be taken if you go to the transit office at 421 Osborne Street. No appointment is necessary.

You may also use your Old Age Security Card when travelling on a Winnipeg Transit bus.
