Manitoba Premises Identification For Livestock and Poultry

Application Guidelines

I. Premises Information

1.*Land Description: A legal land description and/or geo-referenced coordinates must be entered. Enter at least one of
    the following.    

2. *Name or Description of Premises: Provide a name or description for the premises (example “home quarter”, “north
     pasture”, etc.).

3. *Premises Type: Check all descriptions that apply to this premises. If “other” is selected specify the premises type.

4. *Species Type: Select all species that may be present at any time during the year at this premises.
     If “other birds” is selected indicate all species present, including:

  • Ostriches
  • Farmed Quails
  • Pigeons Emus
  • Farmed Geese
  • Exotic or Show Birds
  • Rheas
  • Farmed Pheasants Farmed Ducks
  • Guinea Hens

       If “other” is selected indicate all species present, including:

  • Donkeys
  • Alpacas
  • Fur-Producing Mink
  • Mules
  • Farmed Elk
  • Fur-Producing Fox
  • Llamas
  • Farmed Deer
  • Farmed Rabbits
  • Vicunas
  • Aquaculture
  • Wild Boars
  • Captive Wildlife                         

      For each of the selected species, indicate the maximum number of animals that this location typically
      accommodates.

5. Premises Number: Include the Premises Location Allocator Number assigned to this land location by the Canadian
    Cattle Identification Agency, if applicable.

6. Civic/911 Number: Record the number (up to six digits) on the sign at the end of the driveway at this premises, if
    present.

7. GLN Number: Record the Global Location Number (GLN) Assigned by GS1 Canada to some busineses,if it has been
            assigned to this premises.

II. Contact Information

1(a)* Emergency Contact Information: Supply the name and phone information of the person to be contacted in the
        event of an emergency. This will be the person responsible for the day-to-day operations at this premises. Also
        supply the business/farm name and the complete address where correspondence concerning this premises will be
        received.

1(b)* Relationship of emergency contact to premises: Select the relationship of the Emergency Contact to this
        premises. If the relationship does not appear on the list select “other” and provide the nature of the relationship.

2.   *Owner of Land Information: Complete this section if this information is different from the Emergency Contact
        Information (part 1). Supply the name, phone numbers, mailing address and business/farm name of the owner.

III. Consent to Disclosure of Personal Information and Animal Premises Information
