Manitoba Premises Identification For Livestock and Poultry

Application Guidelines

• Print clearly.
• Required information is denoted by an *.
• The owner or operator of a livestock or poultry premises should apply.
• This application is only to be completed for livestock and poultry premises (see list of species in section I part 4).
• Complete this application for at least one premises, preferably the primary premises. A primary premises is the legal land location on which the main animal operation is situated.
• If applying for more than one premises, attach an additional application for each premises. On additional applications, complete sections I and III. The information from section II (Contact Information) does not need to be supplied on additional applications unless the contact information differs from the information supplied on the original application.
• If providing information on more than five premises contact the local Manitoba Agriculture, Food and Rural Initiatives (MAFRI) Growing Opportunities (GO) office for assistance.

I. Premises Information

1.*Land Description: A legal land description and/or geo-referenced coordinates must be entered. Enter at least one of
    the following.    

•  If entering a legal land description select one quarter, record the section, township, range and select  the west
   or east meridian. If the quarter has been subdivided include the description of the subdivision found on the land
   title.
•  If the legal land description is not a quarter section enter the type, lot and name.
•  If geo-referenced coordinates are known or if the land description does not fit into the above mentioned legal
   land  descriptions, provide geo-referenced coordinates in the form of decimal degrees. A location referenced by
   the coordinates (example sow barn front door) must be listed.

2. *Name or Description of Premises: Provide a name or description for the premises (example “home quarter”, “north
     pasture”, etc.).

3. *Premises Type: Check all descriptions that apply to this premises. If “other” is selected specify the premises type.

4. *Species Type: Select all species that may be present at any time during the year at this premises.
     If “other birds” is selected indicate all species present, including:

• Ostriches
• Farmed Quails
• Pigeons Emus
• Farmed Geese
• Exotic or Show Birds
• Rheas
• Farmed Pheasants Farmed Ducks
• Guinea Hens

       If “other” is selected indicate all species present, including:

• Donkeys
• Alpacas
• Fur-Producing Mink
• Mules
• Farmed Elk
• Fur-Producing Fox
• Llamas
• Farmed Deer
• Farmed Rabbits
• Vicunas
• Aquaculture
• Wild Boars
• Captive Wildlife                         

      For each of the selected species, indicate the maximum number of animals that this location typically
      accommodates.

5. Premises Number: Include the Premises Location Allocator Number assigned to this land location by the Canadian
    Cattle Identification Agency, if applicable.

6. Civic/911 Number: Record the number (up to six digits) on the sign at the end of the driveway at this premises, if
    present.

7. GLN Number: Record the Global Location Number (GLN) Assigned by GS1 Canada to some busineses,if it has been
            assigned to this premises.

II. Contact Information

1(a)* Emergency Contact Information: Supply the name and phone information of the person to be contacted in the
        event of an emergency. This will be the person responsible for the day-to-day operations at this premises. Also
        supply the business/farm name and the complete address where correspondence concerning this premises will be
        received.

1(b)* Relationship of emergency contact to premises: Select the relationship of the Emergency Contact to this
        premises. If the relationship does not appear on the list select “other” and provide the nature of the relationship.

2.   *Owner of Land Information: Complete this section if this information is different from the Emergency Contact
        Information (part 1). Supply the name, phone numbers, mailing address and business/farm name of the owner.

III. Consent to Disclosure of Personal Information and Animal Premises Information

•  Print name, sign and date.
