5. Premises Number: Include the Premises Location Allocator
Number assigned to this land location by the Canadian
Cattle
Identification Agency, if applicable.
6. Civic/911 Number: Record the number (up to six digits) on
the sign at the end of the driveway at this premises, if
present.
7. GLN Number: Record the Global Location Number
(GLN) Assigned by GS1 Canada to some busineses,if it has
been
assigned to this premises.
II. Contact Information
1(a)* Emergency Contact Information: Supply the name
and phone information of the person to be contacted in the
event of an emergency. This will be the person responsible
for the day-to-day operations at this premises. Also
supply
the business/farm name and the complete address where
correspondence concerning this premises will be
received.
1(b)* Relationship of emergency contact to premises: Select
the relationship of the Emergency Contact to this
