
The forms below are the “official” Family Services and Consumer Affairs consent forms (effective April 2008).
The “Consent for Obtaining and Sharing Information for Eligibility and Participation” form is to be used as the front end consent form for what is best described as the Counsellor – Applicant/ Participant Relationship.
The “Consent for Sharing with Service Providers” is to be used when, and only when, making a referral to a third party and information sharing is expected. Typically, these referrals are to Employment and Training Centres and Supported Employment agencies. Although the names of the service providers are not required to be listed on the form the participant should be made aware of who the service provider is and what information will be shared and obtained.
The “Consent for Sharing with Employers” is required to be completed and signed by the participant any time information about the participant will be shared or obtained with or from the employer. The Employer names must be listed.
It is the Vocational Counsellor’s responsibility to ensure participants understand the purpose of their consent and are provided with a copy of all signed consent forms.
Designated Agencies are not required to use these exact forms but they do represent a minimum standard and can be used to guide the development or modification of agency specific consents already in use.
