Appendix IX - Individualized Vocational Plan

1. Shall be completed for all participants within six months of case opening.
2. Must be attached to the initial funding request and when there is a significant amendment to the plan impacting on funding. (See section 111.1 for process and procedural details)
3. Vocational Goal is required for all funding requests except when the funding request if for an assessment that is being used to develop the vocational plan.
4. Participant, Vocational Counsellor and Supervisor signatures are required.
5. Participant receives a copy.
6. Two form designs are provided for Vocational Counsellor preference of use.
