
The Client Identification Form is completed and forwarded to the Community Services Delivery Division, Service Delivery Support Branch, for recording in the Central Registry. This form should be submitted:
Once the Client Identification Form has been entered into the marketAbilities Program Registry database by Service Delivery Support, a hard copy of the Registry data is printed and forwarded to the originating office.
Changes to any of the identifying information or demographic data can be made on either the Client Identification Form or the hard copy of the Client Registry printout. Only key identifying data and the changes highlighted in red are required to amend or update client registry information.
Upon Case Closure the last section of the Client Identification form is required to be filled out and includes reason codes for case closure. The reason codes are as follows:
