Appendix X - Funding Request and Approval Form

1. Official document for requesting and approving funds required for the implementation of an Individual's Vocational Plan.
1. Personal data.
2. Referring agency/regional office data.
3. Financial requests.
4. Official signatures of:
   1. Service Co-ordinator;
   2. Supervisor/Program Manager;
   3. Service Delivery Support (when required)
5. Individualized Vocational Plan Form is required to be attached to the funding request form for initial funding requests or significant changes to the Vocational Plan.
6. Employment Services and Employment Supports forms should be attached to the Funding Request forms for local approval and are not required to be forwarded to SDS unless SDS is responsible for approval.
