Appendix XII - Forms for Employment Supports

Request for Employment Supports - Initial Request
 

1. To be submitted each time a new job commences;
2. Should be attached to a marketAbilities Program Request and Approval  for Employment and Training Centre (ETC) Services and Individualized Support Costs form VR 002-2008);
3. Time frame may be of any duration up to 12 months if Vocational Counsellor plans to maintain contact with participant as an open file. If the Vocational Counsellor plans to close the file then the time frame may be up to 36 months.
4. Maximum number of hours of anticipated services per year is 200. Requests exceeding 200 hours will need to be submitted to Service Delivery Support for Review and Approval.
5. This form is not required to be submitted to Service Delivery Support unless SDS is responsible for approval.

Request for Employment Supports – Extension Request (within first 36 months)

1. To be submitted with a marketAbilities Program Request and Approval  for Employment and Training Centre (ETC) Services and Individualized Support Costs form VR 002-2008)
2. The extension request can be for any duration up to the remaining portion of the first 36 months but is limited to 12 months if the Vocational Counsellor continues to maintain the file as an open case.
3. Should be used to increase hours within current year;
4. This form is not required to be submitted to Service Delivery Support unless SDS is responsible for approval.  (i.e exceeds 200 hours in a 12 month period of time)

Request for Employment Supports - Beyond 36 Months

1. To be submitted with a marketAbilities Program Request and Approval  for Employment and Training Centre (ETC) Services and Individualized Support Costs form VR 002-2008) to Service Delivery Support.
2. Employment Supports may be extended beyond 36 months in situations where the services required are critical for maintenance of employment. The services required to maintain employment must fall within the range of 25 – 200 hours per year.
