Training-on-the-Job - Agreement Form

1. Official contract with an employer specifying the terms, conditions and financial reimbursement for a Training-on-the-Job Agreement.
2. Three copies with original signatures must be submitted with a Funding Request form (VR 001-2006), Training-on-the-Job Initial Request or Request for an Extension.

Training-on-the-Job - Initial Request

1. Must be submitted with a Funding Request form (VR 001-2006) and the Training-on-the-Job Agreement Form.
2. Required for each employment situation should the participant be involved with more than one employer.

Training-on-the-Job - Request for Extension

1. Must be submitted with a Funding Request form (VR 001-2006) and the Training-on-the-Job Agreement Form.
2. Must be submitted two weeks prior to effective date of the extension.
