Chronic Disease Branch

Purpose

Consistent with the Manitoba Healthy and Healthy Living vision and mission, the Chronic Disease Branch works towards a health care system that is responsive to the needs of people with or at risk for chronic disease.  This includes improving and sustaining capacity for chronic disease prevention at community, regional, organizational and government levels.

Activities:

  • Apply evidence to develop, implement and evaluate chronic disease policies and programs.
  • Advance chronic disease policies, processes and systems within and outside government that build capacity (skills, knowledge, structures and resources) to reduce health disparities.
  • Strengthen and maintain a provincial chronic disease surveillance system.
  • Promote integrated, community-based interventions to address chronic disease.

Expected Outcomes:

  • Improvement in the organization and delivery of health care for people with chronic disease, through interprofessional teams and case managers, electronic information systems, self-management supports and client-centered care.
  • Better care for high-risk populations that includes identification of people at risk, and services to prevent and manage chronic disease and associated complications.
  • Co-ordinated and sustained action on prevention that includes a combination of healthy public policy, targeted clinical care, supportive environments and community-based approaches to promote healthy choices in every setting.
  • Surveillance systems that support better tracking, research, public reporting and program/policy development on chronic disease.
