The Family Doctor Finder is designed to connect you and your family with a regular primary care provider- who can provide you with a home base for your health care needs. Here is information to help you understand how the program works. Additional information is also available by clicking here.
If you live in Manitoba and you need a family doctor the Family Doctor Finder can help you find one.
When you register with the Family Doctor Finder you will be asked to provide some basic information, as well as your first and second choices for location and the type of provider (family doctor, nurse practitioner).
Once you are registered you will be given a registration number, and staff in your health region called Primary Care Connectors, whose job it is to find you a doctor, will work to find a provider that is accepting patients in a location that works for you.
There are two ways to register with the program:
Primary Care Connectors are specialized staff in your health region. Their job is to connect people who need a family doctor with a regular primary care provider. Once you register with the program, your regional Primary Care Connector will work with you to find a primary care provider who meets your needs, in a location that works for you.
When you register with the Family Doctor Finder, Manitoba Health, Seniors and Active Living (MHSAL) collects basic information such as your name, date of birth, sex, Personal Health Information Number (PHIN), address and contact information.
MHSAL collects this information from you in order to administer the Family Doctor Finder and connect you to a primary care provider. This information will also be shared with your regional Primary Care Connector so that he/she is aware of your particular preferences, needs, and how to get in touch with you.
MHSAL will protect your information in accordance with the privacy provisions of The Personal Health Information Act (PHIA). Your information is being collected in accordance with Subsections 13(1), 13(2) and 14(1) of PHIA for your registration and participation in the program. For more information on PHIA, please call 204-788-6612 or by email PHIAinfo@gov.mb.ca.
