Filing a Complaint
To register a complaint, download either of the two forms below. Completed forms can be mailed, faxed or delivered to the Law Enforcement Review Agency office.
Fill in online : fill the form in online, then print it out, sign it and submit to LERA
Fill in by hand : print the form out and fill it in by hand. Then sign it and submit to LERA
