Report forms have been developed in accordance with regulations pursuant to the Drinking Water Safety Act. Guidelines for completing the report forms are available in the appendices' of the Operational Guidelines for Monitoring and Reporting - Public and Semi Public Water Systems.

Operating Licence for a Public or Semi-public Water System

The operating licence application form includes a checklist that outlines the documents that must accompany the submission for the application to be considered satisfactory (Section 8 of the Drinking Water Safety Regulation)

Water System owners may request an amendment to an operating licence by submitting the following request form (Subsection 10(1) of the Drinking Water Safety Regulation)

Water System owners are required to renew their operating licence 60 days prior to the expiration date by submitting the following renewal form to the regional Drinking Water Officer (Subsection 14(1) of the Drinking Water Safety Regulation)
