Appendix III - Incident Report

Participants officially enrolled in Employability Assistance for People with Disabilities and in an approved training activity are covered for damages caused to others or the property of others.

 

The Incident Report Form should be completed immediately, whenever an incident occurs that has the potential of resulting in a claim.

 

Instructions

  1. Report all incidents (other than Vehicle Accidents and Workers Compensation claims) involving:
    1. Loss or damage to any Government property over $100.00.
    2. Death or injury to persons.
    3. Damage to property of others.
    Reports must be completed and forwarded immediately without waiting for any letter of claim or writ from involved third parties.
  2. Do not admit liability; forward all correspondence from third parties directly to Insurance and Risk Management Branch.
  3. Do not make statements to any Insurance Company or Adjuster without prior consent of the Insurance and Risk Management Branch.
  4. Do not engage attorneys or experts without prior consent of the Insurance and Risk Management Branch.
  5. Preserve all evidence indicating cause of incident including photographs, if available. Evidence should only be released to law enforcement authorities in exchange for a written receipt.
 

Where a fatality, serious injury or damage to property in excess of $5,000.00 occurs, report immediately by telephone to (204) 945-1917 and follow immediately with written papers:
