Appendix X - Forms for Training-on-the-Job

1. Official contract with an employer specifying the terms, conditions and financial reimbursement for a Training-on-the-Job Agreement.
2. Three copies with original signatures must be submitted with an Individualized Training Funds Request and Approval Form (EAPD-001), Training-on-the-Job Initial Request, or Request for an Extension.

1. Must be submitted with an Individualized Training Funds Request and Approval Form (EAPD-001) and the Training-on-the-Job Agreement Form.
2. Required for each employment situation should the participant be involved with more than one employer.
