Appendix VIII - Individual Vocational Plan
- Shall be completed for all participants within six months of case opening and amended as needed.
- As the Individual Vocational Plan represents an agreement between the client and Employability Assistance for People with Disabilities and is one of the few documents signed by the client, a copy must be maintained in the client file to support funding requests.
- Participant, Vocational Counsellor, and Supervisor signatures are required.
- Participant receives a copy.
- Two form designs are provided for Vocational Counsellor preference.
