Child Abuse Registry Self-Check Application Form

The purpose of the Child Abuse Registry is to help child and family services agencies protect children. Access to the Registry is restricted and all names and information are confidential. Under the Act, you may apply to see if your name is listed on the Registry.

Note: If you are applying for an unpaid position working with children (e.g. a volunteer, student trainee or work placement), please contact the organization and complete their application.

You can download the application form to send in by mail or fax using the following steps:

  1. Download the Application Form (below).
  2. Download and read Child Abuse Registry Check Information for Individuals (PDF Format - 95 KB), which gives information on the Registry, filling out an application and fee payment.
  3. You need to provide a photocopy of 2 pieces of identification, which must be reviewed and signed by someone in a position of authority. You can download the following instructions to provide to the individual: Child Abuse Registry Check Commissioner for Oaths, Notary Public or Professionals (PDF format - 114 KB).
  4. Return your application, certified photocopy of your identification and fee by mail or fax to the Child Abuse Registry at the address below.

Note: Please allow up to six weeks for results of the check.
