Application for Well Licence (pdf)
- A person may make application to the Director under Subsection 90 (1) of The Oil and Gas Act and Section 6 of Drilling and Production Regulation requesting approval for a well licence:
a. The applicant must first have a Business Associate ID. If you do not have one, apply for one here: Petrinex | Minerals, Petroleum and Geoscience | Business, Mining, Trade and Job Creation | Province of Manitoba; along with evidence that the applicant is a duly registered business entity with the Province of Manitoba (please call the Companies Office at 204‐945‐2500 for confirmation);
b. a completed online MEWLA form (please contact Petroleum@gov.mb.ca@gov.mb.ca with your BAID for the link);
c. a cover letter specifying, but not limited to, the contact person, phone no., e‐mail address, anticipated spud date, surface location of the well, anything unusual about the application, etc.(this is for public information). Any surface or mineral information and files should have a separate letter and folder (this is not for public information);
d. documents required from and done by an authorized Land Surveyor, of the area surrounding the proposed well:i. one (1) copy of original survey plat without initials (photo mosaic);
ii. one(1) copy with owner(s) initials; and
iii. grid file in dxf format.e. an executed freehold lease if the oil and gas rights are not owned by the crown;
f. a letter of authorization from the mineral lease holder if the Crown mineral lease is not under your company name;
g. a certificate of title if the applicant owns the oil and gas rights;
h. an executed surface lease to access the wellsite or surface leases if the access road encompasses numerous LSDs in a quarterly sections;
i. a completed ‘Consent of Occupant’ form if the occupant is different than the surface owner (click here for the form);
j. a pooling agreement of owners of oil and gas rights of a spacing unit (if applicable);
k. a farmout agreement (if applicable);
l. assignment of minerals included (if applicable);
m. summary page of multiple lease owners, percentages and area (if applicable);
n. a plan to prevent or minimize the impact to the environment if the well’s location is in an environmentally sensitive area (if applicable);
o. copy of drilling program, including:i. Geological prognosis;
ii.Drilling program (Stick Diagram);
iii. Logging Program (If applicable); and
iv. Stratigraphic testhole drilling and abandonment details(if applicable);
v. Re‐Entry of an Abandoned well (workover program required);
vi. Injection wells (completion program required);p. a licensing fee and levy of $1,000 per well; and
q. a performance deposit of $60,000
- For horizontal well application, please submit also the following in addition to the above mentioned requirements:
a. the bottom hole location of the well and the boundaries of all spacing units that are in the drainage unit of the well;
b. proposed profile of the well; and
c. any applicable production allocation letter(s) signed by mineral owners for 100% freehold wells, freehold and unitized lands wells.
