Appendix VIII - Assessment Information Form

  1. The Assessment Information Form is to be completed by the Vocational Counsellor within three months of case opening.
  2. The form remains on the participant file.
  3. This is a working and confidential file document. The participant may ask to see the content directly or through a FIPPA Request.
  4. For more detailed information on Process and Standards refer to Section 55.
