Apprenticeship Application Agreement (fillable online) - This form is required to begin an apprenticeship and to be registered as an apprentice. It is fillable on-line or it can be printed and completed manually. To submit, the application must be printed and include signatures in ink where specified.

Verification and Disclosure of Manitoba Apprenticeship Training and Certification Credentials
This application is for out-of-province requests for information about Manitoba apprenticeship training and certification credentials and is completed by the government body or person(s) requesting this information.

Verification and Disclosure of Out-of-Province Apprenticeship Training and Certification Credentials
This application is intended for clients who would like their out-of-province credentials assessed for recognition in Manitoba.

Increase in Ratio Application - Apprentice/Journeyperson Training Ratio (fillable online)

Exemption Permit Application-Allows a non-certified employee to perform a portion, but not all of the taskswithin the scope of a compulsory certification trade

Copy of Personal Records Form -To order another copy of a Report of Hours, Pocket Card, Trade Portfolio, Certificate of Qualification or a record for the Trades of Hairstylist, Esthetician and Electrologist.

Notice of Appeal Application

Exam Application: Applicants eligible to write an examination i.e.Trades Qualification Exam, Practical Exam, or an Exam Re-Write will be notified by mail with an Exam Application form. For Level Placement Exams or Provincial certificate holders who would like to challenge the interprovincial exam, contact the Apprenticeship Branch for an Exam Application Form.

Trades Qualification (TQ) Application & Work Experience Forms
(Application for certification through work experience)

To apply for Trades Qualification, ensure you submit the following documents:

  1. Trades Qualification Application (fillable on-line).
    This form fillable on-line or it can be printed and completed manually. To submit, the application must be printed and include signatures in ink where specified.
  2. Work Experience Form for your trade
  3. Employer Letter(s) documenting exact dates of employment, total hours worked, a brief description of duties performed, the employer signature and any trade related certificates. If your employer is no longer in business or if you were self-employed during this time for which you area applying, please submit a Self Declaration Letter instead of the Employer Letter.

After you have submitted the required documents and have been approved to challenge the exam, you'll need to start preparing. Apprenticeship Manitoba's Trades Qualifications office has put together a guide (called the Trades Qualification Learning Supports) to help you prepare for the Interprovincial/Provincial Certification Exam in your trade.

* Effective April 1st, 2009, two separate fee structures have been approved for Trades Qualification resulting in Trades Qualification Application fee of $100.00 and Trades Qualification Examination fee of $250.00.

The grandparenting clause has been re-established for the trade of Steamfitter-Pipefitter until February 21, 2010.

Please complete and submit the following;

Designated Trainer Work Experience Application

This application should be used to apply as a designated trainer for employers of both regular and Senior Years apprentices. Please ensure you submit the following two documents:

Senior Years Apprenticeship Option (SYAO) Applications & Forms

To apply for SYAO, ensure that you submit a SYAO Registration Form, Apprenticeship Application/Agreement, CareerFocus Application (if applicable), a high school transcript, application fee, if required an SYAO Designated Trainer Work Experience Application for trainers that are not certified journeypersons.

Mutual Recognition (AIT)

Application to recognize certificates of qualification for a Manitoba compulsory trade received in another Canadian jurisdiction. Mutual recognition is required for eligibility to work in a compulsory trade in Manitoba. Applications are not required from candidates with a Certificates of Qualification from jurisdictions already approved for Mutual Recognition or for those wishing to work in non compulsory trades.
