What does the benefit offer?

• A monthly income supplement for six months to help you buy healthy foods during pregnancy
• Referral to community Public Health Services and resources
• Support to connect  with prenatal care early in pregnancy
• Referral to a Healthy Baby Community Support program

To receive the benefit, you must apply while you are pregnant. The amount you receive is based on your income, up to a maximum $162.82 a month. If you are receiving income assistance, the Manitoba Prenatal Benefit cheques will not affect your other benefits.

You may qualify if you are:

• Pregnant
• Live in Manitoba
• Have a net family income of less than $32,000 a year

To apply for the Benefit, you need to:

1. Choose an application option that best works for you:
   • Electronic Application Form (E-Form) - Anyone can complete this application form and submit it online. You can upload your note and any other required documents
   • Printable Registration Form (PDF) - If you are currently receiving Employment and Income Assistance (EIA) or Manitoba Supports for Persons with Disabilities (MSPD) you can download, print and submit the shortened Registration Form by mail or in person. Include your pregnancy confirmation note and any other required documents .
   • Printable Application Form (PDF) – If you are not receiving EIA or MSPD, you can download, print and submit the Application Form by mail or in person. Include your note and any other required documents.
   • Application forms can also be picked up in medical offices, community agencies, and government offices, or can be picked up from the Healthy Baby Prenatal Benefit Office: 100-114 Garry Street, Winnipeg, Manitoba.
2. Include a note confirming your pregnancy.

Other Manitoba Prenatal Benefit Forms

• Change in Employment of Family Status (PDF)
  Complete this form if your current year's income is significantly lower than the net family income reported on your most recent tax return.
• Declaration of Income for Minor Applicants (PDF)
  Applicants who are 18 years of age or younger and have not filed an income tax return must complete this form along with the Manitoba Prenatal Benefit application.
• SIN application form
  Applicants who do not have a Social Insurance Number (SIN) must also include a completed SIN application form.
