A Manitoba Health, Seniors and Active Living representative uses a database to record basic demographic and contact information, as well as the person’s preferences for provider type and location.

Primary Care Connectors receive this information and work with primary care providers who are accepting patients to connect program registrants to a provider in their preferred geographic area.

The upgraded program provides an easy way for people without a regular primary care provider to register and be connected with a provider who meets their needs.

How will Primary Care Connectors help me?

Primary Care Connectors are a central point of contact to help connect you to regional and provincial resources. They provide a bridge to new patients without inundating clinics with requests. 

They can maintain regular contact with you and be available to help problem-solve or discuss opportunities to support your practice. 

Primary Care Connectors also connect regularly with provincial and regional programs and services such as emergency departments to identify interested individuals who need a regular primary care provider.
