How does the Family Doctor Finder benefit clients I am seeing?

Connecting people living in Manitoba to a regular primary care provider helps to ensure that they receive ongoing care and support from someone who knows their medical history and health needs.

Having a home clinic that provides continuous coordinated care helps people to improve their overall health and wellness and prevent and manage health conditions.

The upgraded program provides an easy way for people without a regular primary care provider to register and be connected with a provider who meets their needs.

How does the new Family Doctor Finder work?

People living in Manitoba, who need a primary care provider, are able to register with the Family Doctor Finder online OR by calling 204-786-7111 (in Winnipeg) OR toll-free 1-866-690-8260. 

A Manitoba Health, Seniors and Active Living representative uses a database to record basic demographic and contact information, as well as the person’s preferences for provider type (family doctor, nurse practitioner) and location.

Primary Care Connectors receive this information and work with primary care providers who are accepting patients, to connect program registrants to a provider in their preferred geographic area.

How can I help?

Because you routinely connect with the public as part of your day-to-day operations, your program or site may be in an ideal position to help identify people who do not have access to a regular health care provider.

You can help connect people you are seeing to a regular primary care provider, if they don’t have one, by providing them with the contact information for the Family Doctor Finder.  Business size cards, with information on how to access the program, are available to provide to people you are seeing.  You can also help your client with the registration process if necessary.

Is there a Primary Care Connector in my Region?

Yes. Primary Care Connectors have been hired in all health regions.

How can Primary Care Connectors work with me?

Primary Care Connectors are a central point of contact to help people living in Manitoba find a regular primary care provider. They work with primary care practices in their region to connect them to supports that can help providers care for patients and help the system to better meet people's needs.

Primary Care Connectors also work with provincial and local programs and services to encourage identification of people who want and/or need a regular primary care provider. 

Your regional Primary Care Connector may contact you, your program or your team to figure out the best way to work together to connect people you may be seeing to regular care and support.
