The form below is the official Employability Assistance for People with Disabilities consent and replaces all previous consent forms.
- The Application and Consent Form allows for the collection and sharing of participant information required by Employability Assistance for People with Disabilities.
It is the Vocational Counsellor’s responsibility to ensure participants understand the purpose of their consent and are provided with a copy.
Designated Agencies are not required to use this exact form but it does represent a minimum standard and can be used to guide the development or modification of agency specific consents already in use.
